• Leave of Absence
  All Faculty, Lecturers and KAPs and VIGRE Fellows are required to submit a leave of absence request to the Department Chair if you plan to be away from the Department.
  • Short term Leave of Absence
    (7 or less calendar days of absence)

  • Long term Leave of Absence or Sabbatical
    (More than 7 calendar days of absence)
    Note: Leave of Absence requests must be submitted three weeks prior to the start date.

• Undergraduate Grade Distribution Form
  Teaching staff can use this online form to submit their grade distributions to the Mathematics Student ervices Office. It should be filled out at the same time you submit final grades.

  KAPs use this form to apply for a department travel award. KAPs can apply once per year and receive up to $1,000.
